We are seeking a compassionate and dedicated Caregiver Support Specialist to join our team at Mercy Aged and Community Care Ltd. As a Home Care Worker, you will play a vital role in delivering high-quality care and support to our clients, enabling them to live independently at home.

Key Responsibilities:

  • Provide personal care, domestic assistance, assisted shopping, and meal preparation to clients in their own homes.
  • Assess client needs and develop personalized care plans to meet their physical, emotional, and social requirements.
  • Work collaboratively with clients, family members, and healthcare professionals to ensure seamless care coordination.
  • Maintain accurate records of client interactions, care plans, and progress notes.
  • Foster strong relationships with clients and their families, promoting trust, respect, and open communication.

Requirements:

  • Certificate III or IV in Individual Support/Aged Care/Disability Support.
  • Current Victorian Driver's Licence.
  • First Aid Certificate (Level 2).
  • Strong communication, organisational, and time management skills.
  • Able to work independently and as part of a team.

Preferred Qualifications:

  • NDIS Clearance.
  • Additional qualifications in aged care, disability support, or related fields.

What We Offer:

  • A competitive salary range of $65,000 - $80,000 per annum, depending on experience.
