Job Summary
">We are seeking a dedicated and compassionate Care Practitioner to join our team in Brisbane.
">As a Residential Care Officer, you will play a vital role in providing individualised, person-centred care to clients with diverse support needs.
">Main Responsibilities">- ">
- Conduct thorough assessments of client needs and contribute to the development of tailored care and support plans that reflect NDIS objectives and personal preferences.">
- Deliver high-quality personal care and daily living assistance, including support with hygiene, mobility, nutrition, and household tasks, to promote wellbeing and independence.">
- Support clients in accessing healthcare services, community participation opportunities, and structured social programs to encourage inclusion and personal development.">
- Apply effective behavioural and emotional support strategies, providing appropriate responses in situations involving mental health challenges or crisis intervention.">
- Maintain accurate and comprehensive records, case notes, and progress reports in line with organisational policies and NDIS quality and safeguarding standards.">
- Collaborate effectively with families, allied health professionals, and community service providers to ensure coordinated and holistic service delivery.">

- A relevant qualification and a minimum of 1 year of experience in a similar role.">
- Demonstrated understanding of person-centred care principles and quality service standards.">
- Excellent communication skills, empathy, and adaptability to diverse needs.">
