Advanced Care Practitioner

We are seeking highly skilled and motivated professionals to fill the role of Advanced Care Practitioner.
The successful candidate will be responsible for providing high-quality patient care, conducting thorough assessments, developing treatment plans, and collaborating with healthcare teams.
Key Responsibilities- Conduct comprehensive patient evaluations to identify health needs and develop targeted interventions
- Collaborate with multidisciplinary teams to provide coordinated patient care
- Develop and implement individualized treatment plans that address the unique needs of patients
- Stay up-to-date on the latest clinical practices and research findings to ensure delivery of best-in-class care
- Communicate effectively with patients, families, and colleagues to promote seamless care transitions
- Hold current registration as a Registered Nurse or relevant qualification
- Maintain current certification in CPR and Anaphylaxis training
- Hold current Immunisation Certification
- Be eligible to participate under Medicare
