This role involves providing domestic assistance, social support, transportation, meal preparation, personal care, and medication management services. To be successful, you will need to develop strong relationships with clients, families, and other healthcare professionals. You will work collaboratively with experienced care coordinators to provide guidance and support. Your duties will include maintaining accurate records and reports as required. You will also participate in ongoing training and professional development to enhance your skills and knowledge. The ideal candidate will hold a Certificate III in Aged Care or related field (or be in the process of obtaining qualification). They must have a valid driver's license and insured vehicle. The candidate should be able to work independently and as part of a team. Excellent communication and interpersonal skills are essential.
