Job Title

Lead with Purpose

As a Care Community Lead, you will be responsible for providing clinical and operational leadership across our care community.

  • Drive quality care by mentoring and developing a high-performing multidisciplinary team.
  • Ensure services meet program guidelines, quality standards while aligning with organisational strategy.
  • Champion a restorative, wellness and reablement approach to care, ensuring customer outcomes are central to every decision.
  • Build and maintain strong working relationships with aged care leadership, external community care and health providers, and customers/supporters.
  • Drive continuous improvement, quality, and risk management processes to enhance service delivery and staff capability.
What You Will Bring
  • Bachelor of Nursing tertiary qualification.
  • Current registration and practicing certificate with AHPRA.
  • Proven leadership experience in a community aged care or health service setting.
  • Senior case management and/or coordination experience with a strong customer service focus.
  • Excellent knowledge of aged care legislation, standards, and programs.
