Transforming Lives through Community Reintegration

The Community Transition Clinician plays a pivotal role in facilitating the reintegration of individuals with severe mental illness, substance use disorders, and cognitive impairment into community care.
This clinician collaborates with a multidisciplinary team to ensure continuity of care by understanding patients' clinical, psychosocial, and behavioural vulnerabilities.
The Community Transition Team is a statewide service that supports people from correctional centres across Sydney metropolitan and regional NSW, and liaises with all NSW Health Local Health Districts.
Key Responsibilities:- Work collaboratively with the Custodial Mental Health Service, primary health networks, Community Mental Health, and Drug and Alcohol Services to provide comprehensive care.
- Develop mental health care plans and establish necessary therapeutic supports for patients.
- Provide follow-up community care to patients leaving custody.
- For nursing staff: Current unrestricted Authority to Practice as a registered nurse with the Nursing and Midwifery Board of Australia and relevant post-registration qualifications in mental health.
- For allied health professionals: Bachelor or postgraduate degree in a relevant allied health profession and registration with the relevant professional board.
