About the Role

The Anglicare Victoria team is seeking an experienced and skilled Active Outreach Practitioner to join our after-hours program. As a key member of our team, you will be responsible for providing complex case management and support services to our clients, foster carers, and staff.

Key Responsibilities
  • Respond to complex after-hours matters and provide hands-on support to our clients and staff.
  • Work collaboratively with our team to achieve program goals and objectives.
  • Provide expert advice, problem-solving, and support to staff and carers.
  • Support young people in crisis, including providing additional staffing support in our residential youth houses.
  • Transport, police, or medical matters.
About You
  • You have experience working with young people of all ages, particularly those with complex needs.
  • You possess excellent communication and interpersonal skills, with the ability to work effectively with a team.
  • You are calm, resilient, and able to make complex decisions in high-pressure situations.
  • You have a strong understanding of mental health issues, substance abuse, and self-harm.
