We are seeking a compassionate and skilled nurse to join our team in delivering holistic, person-centred care to older adults experiencing health challenges, chronic conditions, and complex needs.

As a registered nurse, you will work as part of a multidisciplinary team to provide early intervention, health education, and recovery-focused support within clients' homes and community settings.

Key Responsibilities
  • Provide direct clinical nursing care to clients in their homes in line with best practice nursing procedures
  • Conduct clinical assessments, dressings, medication assessment and catheter changes
  • Develop holistic nursing care plans in collaboration with client, carer, and allied health where relevant
  • Provide safe, quality nursing practice to clients with specific or complex needs (e.g. wound care, palliative care, education and support)
  • Liaise with all parties involved in the care of the client to maintain holistic healthcare and wellbeing
  • Ensure all documentation is accurate and completed in a professional and timely manner
Requirements
  • A genuine passion for providing high-quality care to the elderly
  • Current AHPRA registration following completion of a Bachelor of Nursing or equivalent
  • Minimum 3 years post-graduate nursing experience
  • Experience in clinical assessments
  • Initiative and the ability to work independently
  • Strong computer skills, especially Microsoft Office products and ability to learn new systems
  • NNSW Driver's license and a reliable vehicle
