Established in 1992, our organisation is committed to securing quality health care for over 6,000 residents across 3 wards.
The NDIS Specialist Support Coordinator role at Miwatj Health presents an exceptional opportunity for an experienced professional to make a meaningful difference in the lives of individuals with disabilities. As a key member of our team, you will oversee and actively manage care plans, ensuring outcomes are met while promoting participant independence and choice.
Key responsibilities include managing a caseload, meeting billable targets, maintaining compliance with NDIS Quality and Safeguarding Practice Standards, and engaging in professional development. This role requires a strong understanding of and commitment to Aboriginal Community Control and self-determination, as well as knowledge of the unique challenges faced by Yolngu people in remote settings.
To succeed in this position, you will need a relevant bachelor's degree (or higher) in disability, health, or social work, experience in Support Coordination or case management within the NDIS, Disability, or Health sector, and an NT Driver's License. Additional checks such as an Ochre Card, NDIS Worker Screening, and a Criminal History Check are required. First Nations candidates are strongly encouraged to apply.
