Miwatj Health is an independent Aboriginal Community-Controlled Health Service. Our mission is to provide culturally appropriate, comprehensive primary health care to remote Aboriginal and Torres Strait Islander residents.
We have a strong commitment to securing quality health care services across the East Arnhem region and enabling Yolŋu powered community health and wellbeing. Our organisation provides health services to over 6,000 remote Aboriginal and Torres Strait Islander residents across 3 East Arnhem wards.
The ideal candidate will have a strong understanding of and commitment to Aboriginal Community Control and self-determination, as well as knowledge of the unique challenges faced by Yolngu people in remote settings.
Job Summary
We are seeking a dedicated NDIS Specialist Support Coordinator to provide culturally safe and efficient specialist case management to NDIS Participants in East Arnhem Land. This role focuses on supporting participants with complex needs to achieve their NDIS goals by overcoming barriers, managing transitions, and connecting with appropriate resources and providers.
You will oversee and actively manage care plans, ensuring outcomes are met while promoting participant independence and choice. Key responsibilities include managing a caseload, meeting billable targets, maintaining compliance with NDIS Quality and Safeguarding Practice Standards, and engaging in professional development.
Key Responsibilities

Required Skills and Qualifications
- Relevant bachelor's degree (or higher) in disability, health, or social work
- Experience in Support Coordination or case management within the NDIS, Disability, or Health sector
- Ability to travel to remote communities
- NT Driver's License
- Ochre Card, NDIS Worker Screening, and Criminal History Check
