As an Aged Care Practitioner, you will play a crucial role in identifying the needs of older adults and connecting them with services that support their independence.

Key Responsibilities
  • Conduct thorough assessments to identify the needs of older adults.
  • Facilitate referrals to essential services that support independent living.
  • Promote the retention and maximization of function and capability in daily life.
  • Work closely with families and carers, recognizing them as key stakeholders.
  • Ensure all assessments are conducted in line with contractual requirements.
What You Need To Succeed
  • At least 1 year of experience in aged care, including client assessments and service delivery.
  • A tertiary education in a health-related discipline, such as nursing, occupational therapy or physiotherapy.
  • Current unrestricted registration with a relevant professional association.
  • Proficiency in Microsoft Office and client management systems.
  • A valid Australian driver's licence and ability to travel within your designated area.
