Wellbeing Support Role

This role is a crucial part of our organisation's mission to provide wellbeing services to vulnerable veterans and their families.

We are seeking an individual with extensive knowledge and experience in Strength-based Case Management, empathy, and working in sensitive environments.

The ideal candidate will possess excellent communication skills, proficiency in Microsoft Office, and the ability to build and maintain relationships with internal and external stakeholders.

The role involves working collaboratively with various groups to improve the quality of life for those in need.

Main Responsibilities:

  1. Develop and implement client support plans using a veteran-centred approach.
  2. Case manage veterans within programs such as Veteran Homelessness and Emergency Relief Funds.
  3. Conduct home and hospital visits to ensure veterans receive essential wellbeing services.
  4. Maintain relationships with internal and external stakeholders to improve service delivery.
  5. Monitor client progress and escalate complex issues when necessary.

Requirements:

To be successful in this role, you will need a degree in Humanities/Arts (Social Welfare), Health Sciences, Social Science, or related field, and 3-5 years' relevant experience.

You will also need to demonstrate strong organisational skills, the ability to work autonomously, and excellent time management skills.
