Job Description

Maintaining accurate and up-to-date records of patient/client interactions and progress is essential in this role.

Candidates should have a bachelor or postgraduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Workers.

Assessing patients/clients' needs and developing appropriate care plans is a critical component of this position.

  • Develop and implement individualized care plans in collaboration with multidisciplinary teams.
  • Provide counseling and support to patients/clients and their families, addressing emotional, social, and psychological needs.
  • Collaborate with healthcare teams to ensure comprehensive care and support.
Requirements

High level written and verbal communication skills and excellent interpersonal skills are essential for success in this role.

Demonstrated knowledge, skills, and clinical experience in Social Work clinical practice is also required.

Bachelor or postgraduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Workers is necessary.

Providing education and training to patients/clients and their families on relevant topics is an important aspect of this position.
