About the Opportunity

We are seeking a highly skilled and compassionate Senior Dementia Care Coordinator to join our team.

The ideal candidate will have a strong background in dementia care and experience in coordinating services for residents living with dementia.

Key Responsibilities
  • To coordinate and manage the delivery of dementia care services, ensuring that residents receive high-quality, person-centered care.
  • To work closely with multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each resident.
  • To provide training and support to staff members on dementia care best practices and strategies for promoting independence and dignity.
  • To collaborate with external partners to access funding and resources to support residents' care needs.
  • To maintain accurate and comprehensive records of resident care plans, progress, and outcomes.
Requirements
  • A degree in nursing or a related field.
  • Current registration as a Registered Nurse with AHPRA.
  • Demonstrated experience working in dementia care, preferably in a leadership or coordination role.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work collaboratively with diverse stakeholders, including residents, families, and healthcare professionals.
  • Knowledge of Commonwealth-funded programs and aged care assessments.
  • Familiarity with ACAT/MyAgedCare processes.
