About the Role

We are seeking a highly skilled and compassionate Carer Support Planner to join our team at Helping Minds. As a key member of our carer support and mental health services team, you will play a vital role in providing carer-focused services and consulting with carers to identify their goals and ensure an appropriate support plan is prepared, implemented, and evaluated.

Key Responsibilities
  • Contribute to the intake, planning, and support function of carers
  • Develop partnerships with local agencies to establish ongoing promotional activity and warm referral processes for carers interacting with those organisations
  • Deliver a Carer-focused service which consults with carers and involves them in service planning, delivery and evaluation to assist in continuous improvement
  • Establish local carer peer support groups, including training volunteers
About You

This position requires a combination of skills, including administration, budgeting, contractor engagement, and some scheduling. You will need to be adaptable to change and flexible in how work is undertaken, with excellent communication skills. A relevant qualification in social work, counselling, community or healthcare services is highly regarded.
