Job Title: Advanced Care Practitioner

In this dynamic and rewarding role, you will have the opportunity to provide exceptional clinical leadership and ensure high-quality care delivery to our residents.

The successful candidate will be responsible for coordinating quality clinical services in line with a resident-centred model of care. This includes managing and providing efficient, effective, and compassionate care planning and assessment, as well as ensuring medication management is provided in a safe and effective manner in line with legislative requirements and policy.

You will work collaboratively with staff, other health professionals, residents, their families, and Clinical Leadership team to ensure clinical decisions are based on best practice and positive resident outcomes.

About You:

To succeed in this role, you will need:

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Demonstrated knowledge and understanding of aged care legislation
  • Commitment to contemporary aged care practice
  • Ability to support and lead a multi-disciplinary team
  • Experience in a clinical leadership role with the ability to manage day-to-day clinical issues
  • Excellent oral and written communication skills
  • Ability to plan, prioritise, and manage day-to-day activities in an efficient manner
  • Demonstrated computer literacy
