To Support Our Continued Growth
At our organization, we seek an enthusiastic and experienced Care Coordinator to help us deliver high-quality support services.
Key Responsibilities
- Coordinate and monitor supports for participants in line with their care plans
- Develop, review, and update person-centred support plans
- Communicate with participants, families, support staff, and external providers
- Manage shift rosters and staff allocation
- Maintain accurate case notes and documentation
- Evaluate compliance with organizational policies
- Respond to incidents and escalate concerns appropriately
About You
- Prior experience in administration or a similar role in disability or community services
- A strong understanding of person-centred care principles
- Excellent communication and organizational skills
- The ability to work independently and as part of a team
- A genuine passion for supporting people with disabilities
Required Qualifications & Checks
- A Certificate IV or higher in Disability, Community Services, or a related field
- An NDIS Worker Screening Check
- A National Police Check (current)
- A First Aid & CPR Certificate
- A Valid Driver's Licence
