About the Role

This is a challenging and rewarding opportunity for an experienced Behaviour Support Practitioner to make a positive impact in the lives of individuals with complex needs.

As a key member of our multidisciplinary team, you will work closely with NDIS participants, their families and/or carers to identify their needs through assessment and person-centred planning.

Your expertise will be utilised to evaluate, develop and deliver evidence-based support plans within a complex support space, ensuring individual plans reflect our NDIS participants' goals, passions and strengths.

Main Responsibilities
  • Assisting NDIS participants, along with their families and/or carers, to identify their needs through assessment and person-centred planning
  • Evaluating, developing and delivering evidence-based support plans within a complex support space
  • Developing meaningful goals and practical strategies utilising a positive behaviour support framework, and with a commitment to eliminate restrictive practices, in the home and community
  • Ensuring individual plans reflect our NDIS participants' goals, passions and strengths
  • Building the knowledge, capacity and strengths of those who support participants in their lives - family, carers, friends, etc
  • Working collaboratively within a multidisciplinary team, including mentoring less experienced Behaviour Support Practitioners
Requirements

To be successful in this role, you will have considerable experience providing support to people with behaviours of concern (professionally and/or personally), excellent report writing skills, valid drivers licence and access to a reliable motor vehicle, and a genuine passion for working in the disability sector.
