Our purpose is to enrich the lives of people living independently in their own homes through connection to services and community.About UsWe are a charity providing care to the elderly to help them live in their own home with dignity while still maintaining their independence.

We provide in-home services 7 days per week for Home Care Package and 5 days per week for CHSP. Our goal is to provide services at the same time, on the same day and with the same team of Support Workers, which means you know the people who are coming and the time they will arrive.

Role DescriptionThe Clinical LeadThe Clinical Lead / Senior Nurse is responsible for supporting complex Home Care Package clients, and delivering high-quality clinical care services.
  • Building and maintaining strong relationships with clients, carers, and families.
  • Developing, implementing, and reviewing individualised care plans to meet client needs.
  • Conducting assessments to identify client requirements and ensuring compliance with relevant standards.
  • Delivering in-home clinical services, such as medication management, wound care, chronic disease management, and palliative care.
  • Acting as a liaison between referral sources, families, and the care team to ensure seamless communication and care delivery.
  • Ensuring compliance with all statutory and organisational policies.
  • Supporting the care team by providing mentorship and encouraging professional growth.
  • Supervising and guiding non-clinical team members to maintain high standards of care.
  • Participating in the on-call roster when required.
