The role of the Clinical Care Partner is integral to our team, supporting clients with complex needs and delivering high-quality clinical care services. Key responsibilities may include:
- Building strong relationships with clients, families, and carers to ensure seamless communication and care delivery.
- Developing and implementing individualized care plans tailored to meet client needs and preferences.
- Conducting thorough assessments to identify client requirements and ensuring compliance with relevant standards and regulations.
- Providing in-home clinical services, including medication management, wound care, chronic disease management, and palliative care.
- Acting as a liaison between referral sources, families, and the care team to facilitate effective communication and care coordination.
- Maintaining compliance with statutory and organizational policies, adhering to industry standards and best practices.
- Supporting the care team by providing mentorship and encouraging professional growth, promoting a culture of continuous learning and development.
- Supervising and guiding non-clinical team members to maintain high standards of care and service delivery.
- Participating in a rotating on-call roster when required, ensuring 24/7 coverage and support for clients and the care team.
- A qualified Registered Nurse with current registration and experience in a care management or coordination role within aged care or community care.
- Strong organizational skills, a proactive approach to problem-solving, and excellent written and verbal communication abilities.
- Minimum 2 years' experience in a similar role, with a strong understanding of aged care and disability sectors.
- Ability to work autonomously and collaboratively within a team, prioritizing client-centered care and achieving positive outcomes.
- Proficiency in care management software and computer systems, with a willingness to adapt to new technologies and workflows.
