Job Title: Clinical Care Partner

Clinical care partners play a vital role in supporting complex Home Care Package clients and delivering high-quality clinical care services.

Key responsibilities include building strong relationships with clients, carers, and families, developing individualised care plans, conducting assessments to identify client needs, and ensuring compliance with relevant standards.

The ideal candidate will be a qualified Registered Nurse with current AHPRA registration, strong organisational skills, excellent written and verbal communication, and experience in a care management/coordination role within aged care or community care.

Job Description

The Role:

  • Supporting complex Home Care Package clients with high-quality clinical care services
  • Building strong relationships with clients, carers, and families
  • Developing individualised care plans to meet client needs
  • Conducting assessments to identify client requirements and ensuring compliance with relevant standards
  • Delivering in-home clinical services such as medication management, wound care, chronic disease management, and palliative care
  • Acting as a liaison between referral sources, families, and the care team to ensure seamless communication and care delivery
  • Ensuring compliance with all statutory and organisational policies
  • Supporting the care team by providing mentorship and encouraging professional growth
  • Supervising and guiding non-clinical team members to maintain high standards of care

Requirements:

  • Qualified Registered Nurse with current AHPRA registration
  • Strong organisational skills and a proactive approach to problem-solving
  • Excellent written and verbal communication skills
  • Minimum 2 years' experience in a care management/coordination role within aged care or community care
  • Ability to work autonomously and collaboratively within a team
