Care Coordinator Physiotherapist

We are seeking a qualified Physiotherapist to join our multidisciplinary team in Residential Aged Care.
As a key member of our team, you will be responsible for providing personalized treatments and care to our residents. Our award-winning Plena Care Pathway has been designed to provide our therapists with an opportunity to tailor therapy programs for our residents, built from resident goals and priorities, and delivers evidence-based mobility and functional programs, care plan reviews, one-to-one therapy interventions and group-based active aging services.
You will have the opportunity to work collaboratively with our experienced team of healthcare professionals to deliver high-quality care and improve resident outcomes. In addition to your clinical expertise, you will also have the opportunity to develop your leadership skills and contribute to the growth and development of our team.

In this role, you will have the opportunity to:
- Provide comprehensive assessments and clinical recommendations to support consumer activities of daily living, mobility, and transfers, falls risk and prevention, equipment requirements, pressure area care and skin integrity management, arthritic joint management, and pain management.
- Demonstrate flexibility and adaptability to the needs of Plena and the aged care facilities, and work within the scope of the Occupational Therapy profession.
Plena Healthcare offers a competitive salary based on experience, ranging from $90,000 to $120,000 per annum, plus a range of benefits including professional development opportunities, paid induction and training, social club and team connection, health kick, recognition programs, ongoing mentoring, treasure chest fund, and referral bonuses.
To be successful in this role, you will need:
- A bachelor's degree in Physiotherapy or equivalent.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Australian working rights.
- Willingness to obtain NDIS worker screening and National Police Check.
