This role is focused on delivering high-quality mental health services to older adults and younger adults with complex needs.
Key Responsibilities- Conduct thorough assessments, develop care plans, and provide treatment for individuals aged 65 and over, as well as younger adults with age-related complexities.
- Collaborate with a multidisciplinary team to deliver recovery-oriented, trauma-informed care that prioritizes person-centered approaches.
- Mentor and support colleagues in a community-based setting, promoting a culture of excellence and teamwork.
Requirements:
- Registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (APRHA).
- Demonstrated commitment to providing recovery-focused, trauma-informed care that respects individual needs and promotes well-being.
- Proven skills in effective triage, assessment, risk management, and care planning, ensuring safe and high-quality outcomes.
