This role offers a unique opportunity for experienced Registered Nurses to transition into a Case Coordinator position, leveraging their clinical expertise to deliver high-quality care to clients with complex needs.

About the Opportunity

In this capacity, you will be responsible for coordinating resources, providing direct client care, and serving as a key escalation point for clinical challenges.

Key Responsibilities
  • Lead and support a dedicated team of clinicians, promoting growth, competence, and emotional wellbeing.
  • Coordinate resources to deliver personalized, evidence-based care that meets client requirements.
  • Deliver hands-on clinical care and resolve complex issues with confidence and compassion.
  • Act as a key escalation point for nurses managing client deterioration and other clinical challenges.
  • Drive effective communication through regular team meetings, stakeholder engagement, and thorough documentation.
  • Support onboarding, mentoring, and student placements to develop a strong, future-ready workforce.
Your Qualifications

We are seeking candidates with:

  • A Bachelor of Nursing with current AHPRA registration.
  • Minimum five years post-graduate nursing experience with demonstrated clinical leadership and resource coordination.
  • Proven ability to meet performance indicators and make innovative decisions that support quality client care.
  • Skilled in mentoring staff and fostering professional development.
  • Excellent communication, negotiation, and problem-solving skills.
  • Comfortable working both autonomously and as part of a collaborative team.
