Opportunity in Aged Care Clinical Assessment

As an Aged Care Clinical Assessor, you will be responsible for conducting thorough assessments to identify the needs of older adults and connecting them with essential services that support independent living.

  • Conduct comprehensive Aged Care Assessments to accurately identify the needs of older adults.
  • Develop and implement personalized care plans to meet the unique needs of each client.
  • Collaborate with multidisciplinary teams to ensure seamless referrals to relevant services.
  • Promote independence and well-being through proactive case management and support.
  • Ensure all assessments are conducted in accordance with relevant regulatory requirements.
Your Qualifications and Skills
  • A minimum of 1 year of experience in aged care, including client assessments and service delivery.
  • Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
  • Current unrestricted General Registration with AHPRA or relevant professional association.
  • Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.
