We are seeking a dedicated individual to conduct thorough assessments and identify the needs of older adults. As a key member of our multi-disciplinary team, you will play a crucial role in connecting them with services that promote independent living.
Key Responsibilities:- Comprehensive Assessments: Conduct detailed evaluations to identify the needs of older adults.
- Service Provision: Facilitate referrals to essential services that support daily living.
- Promote Functionality: Actively promote the retention and maximization of function and capability in daily activities.
- Stakeholder Engagement: Work closely with families and carers, recognizing their vital role in the care process.
- Compliance: Ensure all assessments are conducted in line with contractual requirements.
About You:
- A minimum of 1 year of experience in aged care, including client assessments and service delivery.
- Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
- Current unrestricted General Registration with AHPRA or relevant professional association.
- Proficiency in Microsoft Office applications and client management systems.
- A valid Australian driver's licence and ability to travel within your designated area.
