Support Professional
About the Role:We are seeking a Support Professional to join our team. The successful candidate will play a vital role in providing support services to individuals with disabilities.
Main Responsibilities:- Providing direct care assistance to clients according to their service plan and as directed.
- Monitoring the safety and well-being of clients and reporting any concerns to the Manager.
- Ensuring completion of appropriate documentation, recording relevant information on support provided in client notes and communication books as required.
- Adaptability to changing circumstances and prioritization of work.
- Client-focused and person-centered approach.
- Commitment to social inclusion and valuing diversity.
- Participating in Lifestyle Service Planning for individual clients and monitoring outcomes.
- Collaborating with clients, family/carers, case managers, and other professionals to support clients' individual goals.
