Importance of this Role

The Salvation Army Australia's Homelessness Stream delivers high-quality, person-centered services to individuals at risk or experiencing homelessness. We support adults and families, including those with children, by offering accommodation, case management, outreach assistance, financial support, and connections to other specialized services.

The Salvation Army's 'Street to Home' initiative offers adaptable support to individuals who are rough sleeping or at risk of primary homelessness.

About the Position

We are looking for a values-oriented and proactive Assertive Outreach Worker to collaborate with a team in assisting individuals (out of office) who are facing primary homelessness, rough sleeping, or at risk of primary homelessness. This role involves developing and executing case plans that facilitate their transition to supported accommodation or independent living.

Reporting to the Manager, this is a permanent, part-time position (30.4 hours per week) based in Launceston, Tasmania. Salary and conditions are aligned with the SCHADS Award Level 5.

Your Responsibilities
  • Collaborate with fellow Street to Home Outreach workers as part of a team.
  • Build effective relationships with rough sleepers and individuals at risk of primary homelessness to promote self-determination and social and economic participation.
  • Deliver assertive outreach and support to clients.
  • Conduct ongoing assessments of clients' housing and health issues, needs, and circumstances from initial intake to exit.
  • Create individual case plans in partnership with clients that encompass a variety of interventions addressing their needs and goals.
  • Assist clients in accessing appropriate support services and establishing community connections, ensuring advocacy with service providers when necessary.
Your Qualifications
  • A tertiary qualification in social work, community welfare, community development, or a related discipline.
  • A current Working with Vulnerable People check.
  • A valid Tasmanian Driver Licence.
  • Experience and knowledge in areas such as Primary Homelessness, Poverty, Social Isolation, Domestic Violence, Mental Health, General Health, Exiting Prison, Disabilities, Youth, Legal Issues, and AOD.
