Key Responsibilities
• Oversee medication management in accordance with resident needs and legislative requirements
• Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers
• Lead compliance with ACFI assessments and reviews
• Support and lead the care home clinical team, including coaching, mentoring, and performance management
• Liaise with allied health, medical practitioners, and other services to provide optimal resident care
• Conduct clinical governance audits and incident reviews
About You
We are seeking a highly skilled and experienced Clinical Care Manager who holds current AHPRA registration and a Bachelor of Nursing or equivalent qualification. Postgraduate qualifications in aged care are highly regarded. Your proven track record in delivering quality clinical care within an aged care or acute setting, combined with your excellent leadership and communication skills, make you an ideal candidate for this role.
