As a skilled social worker, you will be responsible for assessing and intervening with inpatients across various locations within the Murrumbidgee Local Health District. Your role will involve providing complex care planning and overcoming psychosocial barriers to discharge or transfer of care from any MLHD facility.

This position requires strong links with external social and community services, particularly those focused on welfare, as well as local transport agencies and funeral homes. You will work closely with patients to support their needs and provide them with access to these essential services.

The successful candidate will report to the Acute Network Lead Manager and work 7 days a week, with opportunities for job sharing and flexible work hours. This role involves weekend coverage and provides a unique opportunity to make a meaningful impact in regional communities.

Key Responsibilities
  • Conduct thorough assessments of inpatients to identify their needs and develop appropriate intervention plans.
  • Collaborate with patients, family members, and other healthcare professionals to ensure seamless care transitions.
  • Establish strong relationships with local service providers to facilitate access to social and community services.
Requirements
  • A tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW).
  • Proven ability to effectively prioritize and organize your caseload.
  • Excellent written, verbal, interpersonal, and negotiation skills.
