Patient Care Coordinator

Are you a dedicated healthcare professional looking to make a meaningful impact in patient care? We are seeking an experienced Registered Nurse to join our dynamic Central Access Unit (CAU) team in the care coordination role.
Care Coordinator Key Responsibilities:- Triage, direct, reject or redirect referrals to community programs
- Monitor clients by telephone and follow up on any clinical issues post discharge from hospital
- Ensure patient care is delivered in accordance with the arranged care plan
To excel in this role, you will require a minimum of 5 years post graduate acute and community experience, along with highly developed problem solving and negotiation skills. Additionally, you should have well-developed interpersonal and communication skills, and be able to work in a fast-paced environment in a flexible way.
