Patient Care Coordinator

In this role, you will work closely with our supportive care team on Ward 8 South, a 24-bed specialist palliative care ward. You will be responsible for providing emotional support to patients and their families, as well as coordinating care activities and resources to ensure the best possible outcomes.
To succeed in this role, you will need excellent communication skills, a strong attention to detail, and the ability to work effectively in a fast-paced environment. You will also need to be able to work independently and as part of a team, with a focus on delivering high-quality patient care.
