Job Summary

We are seeking a dedicated and experienced Case Manager to join our dynamic team. As a Case Manager, you will play a critical role in coordinating patient care and ensuring the highest level of service delivery.

Key Responsibilities:

  • Conduct comprehensive assessments of patient needs and develop personalized care plans.
  • Collaborate with healthcare professionals to optimize patient outcomes and ensure seamless transitions between care settings.
  • Manage referrals from healthcare professionals or agencies and ensure timely communication and coordination.
  • Maintain accurate and up-to-date client records, including care plans, assessments, and communication logs.
  • Ensure compliance with privacy regulations and organizational policies.
  • Participate in continuous quality improvement activities to enhance patient care and service delivery.

Requirements:

  • 1+ year experience as a HCP case manager.
  • Preferred Bachelor's degree in Nursing (or equivalent).
  • Previous experience in care coordination or a related role in the healthcare industry.
  • Strong organizational, communication, and interpersonal skills.
  • Knowledge of home care regulations and industry best practices.
  • Ability to handle and resolve conflicts effectively.
  • Proficient in computer applications, including Microsoft Office and care management software.
  • CPR and First Aid certification.

Preferred Skills:

  • Clinical Expertise: Proven experience in providing direct client care and ensuring safety.
  • Government Funding HCP /CHSP: Good understanding of government funding for aged care.
  • Follow Up/Liaison: Ability to follow up with hospitals or health professionals as needed.
  • Health Assessments: Good understanding of health assessments and developing care plans.
  • Multitasking and Time Management: Ability to multitask and manage time effectively.
  • Communication: Excellent verbal and written communication skills.
