Lead Community Care Services

We are seeking a seasoned Registered Nurse to assume a leadership role in community-based aged care services.

This position offers the opportunity to influence and lead across Support at Home, Home Care Packages, CHSP, and community-based TCP programs from our Pennington-based Head Office.

  • Provide strategic guidance and clinical oversight to a multidisciplinary team of Care Managers (including RNs, ENs, and non-clinicians)
  • Drive the delivery of safe, effective, and person-centred care across all community programs
  • Support complex client care planning, reassessment, and clinical escalation processes
  • Monitor and respond to clinical risks, incidents, and compliance requirements
  • Ensure adherence to Aged Care Quality Standards, the Aged Care Act, and Support at Home reforms
  • Support onboarding, mentoring, and professional development of frontline care staff
  • Act as a key clinical interface with external providers, allied health, and regulatory bodies

Requirements:

  • Current registration as a Registered Nurse Division 1 with AHPRA
  • Demonstrated experience in aged care, community care, or home care
  • Proven clinical leadership capability – including staff supervision, escalation of risk, and multidisciplinary collaboration
  • Strong understanding of care planning, clinical governance, and regulatory frameworks
  • Excellent verbal and written communication skills
  • Comfortable working in digital care systems (experience with eCase or similar is a bonus)
  • Ability to work independently while contributing to a collaborative, values-driven team
