Job Title: Community Care Team Lead

We are currently searching for an experienced team leader to help guide our field team day to day. The successful candidate will be responsible for ensuring that clients' needs are met within the required timeframes and parameters.

The ideal candidate will have:

  • A Certificate IV in Community Services or equivalent
  • Excellent leadership and supervision capabilities
  • Problem-solving and decision-making skills with the ability to think on your feet
  • Highly developed assessment skills and the ability to creatively problem solve care situations

In this role, you will work closely with our management team and other stakeholders to ensure that the team works in a flexible and collaborative manner to deliver positive client outcomes.

You will also be responsible for providing line supervision and support for a team of Personal Care Workers (PCW), ensuring they provide services at best practice standard and aligned to our values and Code of Conduct.

Key Responsibilities include:

  • Providing line supervision and support for PCWs
  • Working closely with the Operations Manager and other key stakeholders to plan for future workforce forecasts
  • Actively monitoring caseloads, new referrals, and allocations
