Job Overview

Ensuring comprehensive management plans are achieved for clients is crucial in our industry.

As a key member of our team, you will be responsible for managing client-centered care plans.

  • Conducting initial phone assessments, creating and implementing individualized care plans, and setting achievable goals
  • Organizing and coordinating home-based services to support clients receiving care at home
  • Building effective relationships with stakeholders to ensure seamless communication and collaboration
  • Regularly monitoring patient progress and updating care plans as necessary
  • Facilitating liaison between healthcare professionals and specialists to provide cohesive care
  • Discharge planning including referrals to long-term services
  • Providing telephone support, education, and advice to clients and their families
