Occupational Therapist Level 3 - Transitional Aged Care Program

The Transitional Aged Care Program is a Commonwealth-funded service that supports older people leaving hospital. As an Occupational Therapist, you will work closely with the multidisciplinary team to help clients achieve their goals and transition from hospital back into the community.

Your role will involve conducting home visits and supporting clients to maintain independence and quality of life. You will assess clients' needs and develop individualized care plans to help them improve their functional abilities.

You will also provide occupational therapy interventions to help clients achieve their goals and maintain independence. This may include conducting assessments, developing treatment plans, and providing therapy sessions.

In this role, you will be part of a supportive and collaborative environment that includes Administration, Allied Health, Podiatry, and Psychogeriatric & Dementia Support teams. You will have the opportunity to work with older adults in their homes, providing therapy, guidance, and support to help them improve their independence and achieve their goals.

  • Conduct home visits to assess clients' needs and develop individualized care plans.
  • Provide occupational therapy interventions to help clients achieve their goals and maintain independence.
  • Work collaboratively with the multidisciplinary team to ensure comprehensive care for clients.

Key Requirements:
  • Bachelor of Occupational Therapy or equivalent qualification.
