About The Role
This Occupational Therapist position is part of the Transitional Aged Care Program (TACP), a short-term (up to 12 weeks) Commonwealth-funded service supporting older people leaving hospital. The program assists clients who may otherwise require residential aged care but who have the potential to improve their physical, cognitive, or emotional function and wish to remain at home.
Based in Taree within Aged Care Services, our team supports the Lower Mid North Coast region. As the TACP Occupational Therapist, you will work closely with the multidisciplinary team to help clients achieve their goals and safely transition from hospital back into the community. This includes conducting home visits and supporting clients to maintain independence and quality of life.
In this role, you will be well supported by the NUM of TACP & Aged Care Assessment, as well as the Service Manager of Aged Care & Generalist Services. You'll be part of a collaborative service that includes Administration, Allied Health, Podiatry, and Psychogeriatric & Dementia Support teams, ensuring a strong network around you. About You
We are seeking a personable and dedicated Occupational Therapist who is passionate about delivering high-quality, patient-focused care. You will bring strong interpersonal skills that enable you to work effectively with older adults in their homes, providing therapy, guidance, and support to help them improve independence and achieve their goals in line with TACP Guidelines.
The successful candidate will be flexible, collaborative, and an excellent communicator, both verbally and in writing. You will thrive in a team environment while adapting to the varied needs of clients. As a Level 3 Occupational Therapist, you will also play a key role in supporting and mentoring junior Allied Health staff, contributing to the growth and strength of the team.
