About the Role:

  • Support the implementation of the National Disability Insurance Scheme (NDIS) through collaborative work with Local Area Coordination, Community and Early Childhood teams.
  • Provide information linkage and capacity building support to communities, enhancing awareness, capacity, and opportunities for people with disability to participate as active citizens.

Key Responsibilities:

  • Develop individual and family capacity to clarify goals, exercise choice and control, and participate in the community.
  • Complete planning pathway tasks with participants in a timely manner, adhering to NDIA practices.
  • Engage with service providers and support people with disability to access their services.
  • Operate within organisational policies and procedures, upholding quality standards.
  • Maintain a cohesive team environment, participating in staff meetings, team meetings, and staff development activities.
  • Contribute to a workplace free from discrimination, harassment, and bullying.
  • Measure and review participant outcomes within the Organisational Outcomes Framework.
  • Participate in meaningful community engagement and capacity building initiatives.
  • Employ effective written and verbal communication skills, including computer literacy and proficient use of IT systems.
  • Utilise excellent organisational, time management, and self-direction skills.
