As a Personal Care Worker, you will co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences. You will work within a multidisciplinary team to provide effective clinical and emotional support to residents, recognising changes in residents' conditions and escalating observations and concerns.
- You will deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
- You will provide personal care to residents with complex needs such as palliative care, dementia support, restorative care and pain management to enhance their wellbeing
- You will support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
- You will monitor the cleanliness, safety, environment and promptly report issues of concern
- Certificate III in Individual Support, or equivalent (Aged Care)
- You are medication endorsed
- Experience in residential aged care/home care (desired)
- Experience with dementia care and palliative care support (desired)
- Demonstrated abilities to support residents in a fast paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners

- A current Working With Children Check card
- Current and ongoing flu vaccinations
- Australian working rights
- Ability to pass a national police clearance and NDIS Clearance
