Job Summary

We are seeking a Personal Care Worker to join our team at Uniting AgeWell. As a Personal Care Worker, you will be responsible for coordinating personal care for residents in accordance with care plans that meet their individual needs and preferences.

Key Responsibilities
  • Deliver high-quality, person-centered care to residents and families, including consideration for cultural, spiritual and individual needs
  • Provide personal care to residents with complex needs, such as palliative care, dementia support, restorative care and pain management
  • Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
  • Monitor the cleanliness, safety, environment and promptly report issues of concern
Requirements
  • Certificate III in Individual Support, or equivalent (Aged Care)
  • Experience in residential aged care/home care (desired)
  • Experience with dementia care and palliative care support (desired)
  • Demonstrated abilities to support residents in a fast-paced environment with exemplary communication skills in a team and one-on-one interactions with all levels of care partners
