As a Housing and Wellbeing Case Manager, you will work with individuals and families in partnership to empower and build their agency to make effective change and overcome their identified housing and wellbeing needs.
This role involves working with clients in crisis accommodation, street outreach and refuges, and building strong ongoing partnerships.
Duties and Responsibilities- Provide high quality case management practices and record keeping that adhere to operational requirements.
- Facilitate client-led assessments and Care Plans to identify client goals, including the whole family, where appropriate.
- Provide role-specific targeted interventions and family facilitated group meetings in achieving client goals.
Benefits
You will have access to a supportive and collaborative team of dedicated professionals, offering a full-time permanent role with a starting salary of $81,000 per annum (depending on qualification and experience) + 11.5% Super + Not for Profit Salary Packaging (Increases your take home pay).
