**Job Title:** Care Team Coordinator


We are seeking a dedicated and compassionate Care Team Coordinator to join our team. As a key member of our care delivery team, you will play a vital role in ensuring the highest level of support for our clients.


**Key Responsibilities:

  • Coordinate care services to meet client needs
  • SUPPORT AND MENTOR Home Care Workers
  • Work with healthcare professionals and families to deliver quality care
  • Ensure services align with best practice and client requirements
  • Develop strong relationships with clients and their caregivers

**Requirements:

  • Certificate III in Individual Support (Ageing) or similar qualification
  • At least 1 year of experience leading a care or support team
  • Current driver's license and insured vehicle
  • Current First Aid and CPR certification
  • Knowledge of Home Care Packages
