Job Description

We are seeking a skilled Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters, gathering clinical information to support the hospice diagnosis and prognosis. This role involves communicating findings to the hospice physician responsible for certifying or recertifying patient eligibility.

  • Conducts physical assessments for F2F encounters within regulatory time frames established by CMS.
  • Provides clinical findings, including medical treatment plans, diagnostic tests, lab results, and medication lists, to the recertifying physician.
  • Makes recommendations to the Interdisciplinary Group (IDG) based on F2F assessment findings.
  • Notifies the hospice agency when unable to complete a scheduled F2F visit.
Requirements
  • Master's or higher degree in Nursing.
  • Graduate-level nurse practitioner education program accreditation by a national accrediting body.
  • Current state license as a registered nurse.
  • Current state license or certificate as an advanced practice nurse.
  • National certification as a Nurse Practitioner (ANCC or AANP).
  • Collaborative agreement in place.
