As a hospice RN/Case Manager with Gentiva, you will have the opportunity to make a meaningful difference in the lives of others. You will work closely with patients, families, and other healthcare professionals to develop and implement personalized care plans that address the physical, emotional, and spiritual needs of each individual.
About the Role- Provide direct patient care services, including administering medications, wound care, and symptom management
- Conduct comprehensive assessments of patients' needs and develop personalized care plans
- Collaborate with interdisciplinary teams to ensure seamless coordination of care
- Communicate effectively with patients, families, and healthcare providers to ensure accurate and timely information sharing
- Maintain accurate and up-to-date clinical records
- RN license in good standing
- 1+ year of experience in hospice or home health care
- Strong communication and interpersonal skills
- Able to work independently and as part of a team
