Job Title: Care Service Delivery Specialist

We are seeking a caring and capable individual to fill a key role in our organisation. As a vital member of the team, you will play a significant part in delivering high-quality care services to clients receiving in-home support.

About the Role:

The primary responsibilities include:

Requirements:

To be considered for this role, you must have:

  • Certificate IV or Diploma in Aged Care, Disability, Community Services, or similar (preferred), with a minimum of 2 years' experience in a related field.
  • Knowledge of Home Care Packages (HCP) and/or NDIS (desirable), with a solid understanding of the sector's regulations and guidelines.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients, carers, and colleagues.
  • Problem-solving and critical thinking skills, with a ability to analyse complex situations and develop effective solutions.
  • Attention to detail and documentation accuracy, with a commitment to maintaining high standards of care and service delivery.
  • Knowledge of community/aged care systems, with a understanding of the sector's complexities and nuances.
  • Leadership and team coordination skills, with the ability to work collaboratively and manage competing priorities.
