Job Title: In-Home Care Specialist

About the Role:

We are seeking a skilled and compassionate In-Home Care Specialist to join our team. As an In-Home Care Specialist, you will provide high-quality care services to our valued clients in their own homes.

Your Key Responsibilities:

  • Deliver consumer-directed care services to clients as prescribed in their care plans.
  • Assist clients with mobility and transfers, as well as other activities of daily living (ADLs) to help maintain their independence.
  • Support clients with general domestic duties.
  • Participate actively in care plan development to ensure clients' ever-changing needs are met.
  • Recognize and respond to changes in clients' conditions, documenting and communicating this information as required.
  • Treat clients with dignity and respect, supporting them to exercise choice and independence.

Requirements:

  • Certificate III qualification in Individual Support, Aged Care, or Community Services.
  • Preferably 12 months experience working in the Home Care sector, particularly with Home Care Packages and CHSP-funded care services.
  • Willingness to perform domestic care duties as part of your role.
  • Genuine empathy with elderly clients and a passion for working within the community service industry.
  • Understanding of specialized service delivery within diverse groups, including Aboriginal and Torres Strait Islander, CALD, Veterans, Disability, Mental Health, LGBTI+, and Homelessness.
