Rehabilitation Social Work Specialist

As a key member of our multidisciplinary Mental Health Rehabilitation Unit team, you will deliver expert clinical social work rehabilitation services to individuals. You will create personalized recovery plans in collaboration with the consumer, their family, and other team members, aimed at helping consumers achieve positive outcomes within the unit and broader community.

About the Role
  • You will utilize advanced social work expertise and discipline-specific skills, such as holistic practices, problem-solving, and family work, to maximize individuals' recovery journeys.
  • You will support consumers in developing daily activities and structures to enhance independent living skills by accessing relevant leisure, vocational, and educational resources.
  • You will collaborate effectively with mental health professionals and service providers to facilitate integrated care planning and provide opportunities for positive rehabilitation and recovery.
  • You will work proactively with families and carers to ensure their input is recognized, their needs are addressed, and opportunities for involvement in program planning are provided.
About You
  • You must hold a degree or equivalent qualification in Social Work which confers eligibility for full membership of the Australian Association of Social Workers.
  • You must have significant experience working within a mental health setting.
  • You must possess skills in conducting comprehensive bio-psychosocial assessments, including risk evaluations of consumers with high or complex needs.
  • You must demonstrate advanced ability in the planning, coordination, and implementation of comprehensive care/service plans.
