Lead Expert Care in Aged Care and Acute Stroke Services

About the Role:

We are seeking an experienced social work professional to lead our team in delivering high-quality care to patients with acute stroke and aged care needs. As a Team Leader, you will be responsible for managing and developing a high-performing clinical social work team, fostering quality improvement through training and collaboration.

  • Key Responsibilities:
  • Deliver advanced, complex clinical services with autonomy.
  • Lead planning, evaluation, and reporting in aged care.
  • Shape protocols, policies, and initiatives at our hospital.
  • Collaborate with multidisciplinary teams for exceptional outcomes.
  • Drive impactful change in stroke and aged care services.

Requirements:

  • Qualification: Bachelor of Social Work
  • Registration: Eligibility for membership of the Australian Association of Social Work (AASW)
  • Tertiary Qualifications: Social Work, eligibility for membership of the Australian Association of Social Work (AASW)
  • Team Leadership Skills: Extensive experience in staff and student supervision.
  • Clinical Experience: High-level clinical experience in aged care social work services, understanding of age-related illnesses, elder abuse, guardianship matters, NDIS matters, grief and bereavement, accommodation placements, and complex familial presentations.
  • Policies and Procedures: Experience in developing and implementing policies and procedures for service improvement, skills in providing training and education.
  • Communication and Negotiation: Well-developed time management skills, excellent written and verbal communication, report writing skills, negotiation, decision making, and advocacy skills.
  • Quality Improvement: Ability to initiate, lead, complete, and implement quality improvement initiatives and service evaluation processes.
  • Licenses: Unrestricted NSW Driver's Licence.
