Care Coordinator

This is an exciting opportunity to join a multidisciplinary team providing personalized treatments to clients in residential aged care and community settings.
As an occupational therapist, you will be involved in delivering high-quality services to consumers referred through government-funded models, private funding, and other sources. You will conduct comprehensive assessments, provide clinical recommendations, and work collaboratively with other healthcare professionals to support the needs of your clients.
- Deliver evidence-based occupational therapy services to clients in their homes or in our facilities, including assessment, treatment, and education.
- Develop and implement individualized plans of care that meet the unique needs and goals of each client.
- Collaborate with other healthcare professionals, including physiotherapists, speech pathologists, and dietitians, to ensure comprehensive care for our clients.
- Bachelor's degree in Occupational Therapy or related field.
- Current AHPRA registration as an occupational therapist.
- Australian work rights and willingness to obtain NDIS screening and police check.
